This is for information of all concerned that a Mediclaim Policy (Health Insurance) for the eligible students of Tezpur University has been renewed with United India Insurance Company Limited, Tezpur, w.e.f. 26^th of February, 2016 for a period of 6 months vide Mediclaim Policy No. 1307002815P114512558. The list of all students eligible for the claim is attached herewith.  Students are advised to verify their names in the list and report to the undersigned if any discrepancy is observed. The procedure to be followed by the patient for the claim of the bill is given below:

The insurance policy covers medical and surgical expenses of students incurred for inpatient (hospitalization) treatment in Hospitals and Nursing Homes for illness and inquiry, i.e. the policy covers the expenses of medicine, hospital accommodation, nursing expenses. Surgeon and physician fees and diagnostic expenses incurred within India for treatment of diseases, subject to the limit of the sum insured of individuals. However, certain diseases like Asthma, Bronchitis, Chronic diseases, Diabetis, Epilepsy, Influenza, Cough, Cold, all Psychiatric Pyrexia of unknown origin for less than 10days, Tonsillitis and upper Respiratory Tract  infection including Laryngitis, Pharyngitis are not covered under Domiciliary hospitalization benefit . All the congenital defect and child birth in case of women are also not covered under this policy. The benefit of this policy is extended as cashless service provided the treatment is taken in accredited hospitals/nursing homes of Tezpur town namely B.K. Memorial Hospital and EMM Hospital and Research Centre.

If a student falls sick outside Tezpur and not admitted in the above hospitals/ nursing homes, he/ she will have to pay the hospital bill himself/ herself.  However, the student may submit his/ her claim for reimbursement to the United India Insurance Company Limited, Tezpur Branch, M.C. Road, Tezpur through the Controller of Examinations. The student will have to sign on the reverse age of each bill and put his/ her bank account no.  in presence of the Controller of Examinations or his representative, who will in turn attest the signature and forward the bill to the Insurance Company for reimbursement.

The Company will reimburse accommodation and nursing expenses to the amount not exceeding 1% of sum insured per day or actual amount whichever is less.

The Insurance Company will reimburse the bill claimed by the patient within 15 days from the date of submission of the same.
